Change is part of our everyday lives; taking on the challenge of change can be essential to making the most of our lives. Our Transitional Support team can help you overcome these challenging situations and help you identify solutions, however large or small they may be.
Transition planning explores what you want to do and how you can go about doing it. The planning helps to break the challenges into small, achievable targets and goals. As each target is reached, you will be moving safely and securely to reach the end goal, where you can say, "This is where I want to be."
At Enham, we work with individuals to develop their person centred plan. Here are just a few examples of transitions:
- moving from a younger adult environment into an adult environment
- acquiring a disability after living an independent life
- wishing to enter a college of further education
- wishing to enter employment
- wishing to manage finances
- wanting a social life and social opportunities
- living in a care home environment and wanting to move to a more independent living environment
